You appear to be on the 17.12.1 AMD drivers. We've been seeing reports of players crashing with this driver. The current workaround for the time being is to downgrade to the 17.11.4 drivers found here. You will want to remove the current drivers with Display Driver Uninstaller before installing the older drivers.

Could you please post in a separate new thread? Add your DxDiag info as well so we can take a look into what may be causing this for you.
You can find this information in the Direct X Diagnostic Tool:
Press Windows Key + R.
Type DxDiag and press Enter.
In the DxDiag window, at the bottom, click Save All Information.
Name the file "dxdiag" and click Save. (note where it's been saved so you can find it)
Locate the Dxdiag.txt file just created and open it to copy and paste ALL of it's contents into your post.
